Senior Care Program

This program provides case management and funding to seniors at risk of nursing home placement.

Eligibility:

• Age 65 or older
• Meet Income and Asset Requirements

Program Services may include:

• Case Management Services
• Financial Assistance for Medications
• Financial Assistance for Incontinent Supplies and Nutritional Supplements
• Financial Assistance for Safety Modifications in the Home
• Financial Assistance for Vision and Dental Care
• Assistance with Personal Care
• Personal Emergency Response Systems
